    The study aims to implement and evaluate the Goals of Care Initiative which is designed to empower cancer patients, who are considering systemic treatment for advanced cancer, to be involved in the decision making process with clinicians. To achieve this, the initiative helps patients & clinicians consider which of their offered treatment pathways is best suited to help them achieve their identified goals, values and/or milestones.

    The Goals of Care Initiative will be implemented by seven oncology teams within a tertiary cancer treatment centre at different time points over the course of approximately 2 years. Once the initiative is available within an oncology team, their patients will be able to access the GOCI resources, which include a website, booklet and DVD, and clinicians will receive appropriate training.

    The evaluation will take place over the same two years and will recruit eligible participants from all of the oncology teams. Patients will be recruited following consultations with their clinical team where treatment options are being discussed and will be asked to complete questionnaires following the consultation, 6 weeks later and 12 weeks later. The questionnaires will assess perceived involvement in shared decision making, decision conflict, perceived ability to make a decision and healthcare service use. Quality of life will also be assessed for health economic purposes. Patients who are recruited prior to the initiative being implemented with their clinical team will act as the control group as per a standard randomised controlled trial, whereas patients who are recruited after the initiative has been introduced into their team will form part of the intervention group.

    Interviews will also be conducted with a sample of patients to determine whether the resources had an effect on their consultations. Clinician and GP views of the initiative will also be collected.
