    Disability is the main consequence of the concurrence of three conditions in older adults: ageing, lifestyle and health-related conditions. It is usually preceded by a state characterised by decreased capacity to respond to demands caused by diminishing functional reserve and this state is called frailty. When the condtion of frailty has been surpassed and disability has emerged recovery is unlikely. However, frailty itself is reversible both spontaneously and through physical exercise. The aim of this study is to assess the feasibility and effectiveness of a programme designed to detect and manage frail older patients in high risk clinical settings and to avoid functional impairment and other associated adverse outcomes. In this interventional phase we aim to compare integrating a care plan by a Geriatrician compared to standaqrd clinical care into the care of frail older patients.
    The study is a randomised multicentre phase III study evaluating frail patients 75 years and over in high risk clinical settings. It is being carried out in three hospital settings in the UK - accident and emergency, general surgery and cardiology.
