    Type 2 diabetes is a common condition that affects blood sugar control and can lead to serious health problems such as heart disease, kidney damage, and sight loss. Most people with type 2 diabetes in the UK are managed in general practice. Many are not on insulin but still struggle to keep their blood sugar within recommended ranges.

    A device called a continuous glucose monitor (CGM) can help. A CGM is a small sensor, usually worn on the arm, that measures sugar levels every few minutes. It shows how food, physical activity, sleep, and stress affect diabetes. While CGM is widely used for people with type 1 diabetes or those on insulin, we do not yet know the best way to use it for people with type 2 diabetes who are not on insulin.

    This study (called FOCUS-CGM) will test two ways of introducing CGM in primary care:

    Self-directed learning: patients see their glucose results in real time and learn from them straight away.

    Clinician-facilitated learning: patients wear the sensor for 15 days but do not look at the results. A clinician reviews the data, identifies key patterns, and explains them at a follow-up visit. Patients then wear a second sensor (another 15 days) to apply the advice while seeing their results.

    We aim to find out whether both strategies are practical, acceptable to patients and staff, and suitable for use in routine NHS care. Around 30 adults with type 2 diabetes (not on insulin) will take part at Country Park Practice in London. Each participant will wear one or two sensors, complete short questionnaires, and attend follow-up visits.

    This pilot study will last about 6 months. Sensors are provided free of charge by Abbott. Findings will guide a larger trial on how best to use CGM in UK general practice.
