  • Research summary

England has an ageing population with older people increasingly experiencing frailty (Alderwick et al 2015, NHS England 2014). Frailty is a distinctive health state related to ageing that can cause complex problems and reduce well being (Age UK 2015). Health care in contemporary policy not only has a focus on delivery in the community setting, where district nurses hold a key role, but also on patient centred care (DH 2010, Woods et al 2015). District nurses provide for a variety of care needs to people living at home (QNI 2015), care can have many meanings in different contexts (Bassett 2004). Concepts of person centred care highlight that people are not passive recipients of care and that services such as district nursing should work together to coproduce effective high quality care such as those in care (NHS England 2014). This study aims to explore the nature of care that results when district nurses and frail older people work together.
The study design is a three phase focussed ethnography. This includes a sequence of an interview, an observation of a care interaction between a district nurse and a frail older person at home, followed by a second interview, all of which will be spread over a year long period. Interviews will be held in the workplace for nurses and the home for older people participants. There will be a total of four district nurse and four older people participants in the study. District nurse participants will have been qualified specialist community practitioners of at least six months duration and will be recruited from an NHS community provider organisation. Subsequently older people will be recruited from district nursing caseloads, following inclusion criteria related to age and frailty status.
The research is non-funded as it is a student project.
