  This project will evaluate the Whole Systems Integrated Care (WSIC) initiative in West London CCG. WISC introduces a new care model for adults aged over 65 in West London CCG. The aim of the model is to provide a planned, proactive, preventative integrated health and social care service to older adults in West London CCG.

  The aim of this project - To provide evidence that the WSIC principles are embedded in the delivery of care across the whole system of provision in WISC

  The research will have three parts.

  1. Audio/video recordings and researcher observation of patient consultations.
  2. Semi-structured interviews with patients
  3. Semi-structured interviews with members of staff
