• Data collection arrangements

  The database will contain basic information about patients who have agreed to be considered for future research projects and are happy to be contacted if they are suitable to volunteer. The information will include;

  1. Name
  2. Date of birth
  3. Hospital number
  4. Contact details (address, telephone number and e-mail address)
  5. GP details
  6. History of hearing problem? YES/NO
  7. Diagnosis right ear
  8. Diagnosis left ear
  9. Copy of latest pure tone audiogram and tympanogram
  10. Medical history - Diagnoses and dates only
  11. Current medication
  12. Allergy status
  13. Smoking status

  The data will be collected after the patients have provided informed consent from the patients NHS health records. Agreeing to be part of the database does not mean that the patient has agreed to take part in research, it only means they are happy to be asked to do so.

• Research programme

  The database will enable researchers from Central Manchester NHS Foundation Trust and the University of Manchester to quickly identify and contact volunteers who could help them complete their research. Estimating the number of patients that are suitable and could be recruited for a specific research project is an essential part of the planning and gaining permission and funding to complete the project. This database will also make this process simpler.
