Pilot Study
To test the feasibility and acceptability of a secondary care place-based intervention focusing on four dimensions linked to the Labonte model.1. Physiological impacts – In the form of a Health Check

4. Wider determinants of health – Providing community and public service assets support.Background
A recent report, "Building a Fairer Gwent: Improving Health Equity and the Social Determinants," calls for tailored interventions to reduce health inequalities.
As part of this pilot study, 20 participants diagnosed with transient ischemic attack, peripheral artery disease, or ischemic stroke (with an NIHSS score of 5 or below) will be eligible to partake in a 12-week intervention programme.
The EQUALITY HEALTH-CVD Intervention offers participants four home visits over 12 weeks.
This involves
1. Consent and Initial Assessments:
Participants will complete a case report form and questionnaires. These will be completed by a research nurse or the researcher
2. First Home Visit: With Equality Health Advisor
A coaching session based on the Labonte model using a “what matters” approach to set personal goals.
Discussions on accessing local community activities or public services.
A physical health check
3. Follow up Visits: With Equality Health Advisor
Three additional visits over the 12 weeks will including a continuation of the coaching session from week 1 and additional support in accessing local community activities or public services
Final visit includes re-completing initial questionnaires and a physical health check
Between 8-10 participants will complete a semi-structured interview.
At the end of the last follow up per Equality Health Advisor, the Equality Health Advisor will be offered to partake in a semi structured interviewREC name
