Research summary
Hospital care for stroke has improved greatly over the last decades but care after discharge from hospital is characterised by lack of continuity, inequalities in access to services and low patient and carer satisfaction. Stroke survivors report a lack of good information about stroke and how to manage it for years after the stroke. Yet around half of stroke survivors report long term unmet needs; one third are depressed, have cognitive problems, poor mobility and reduced quality of life. One third also have at least three other diseases. With a changing population and radical changes to health and social care we need to develop cost effective ways of improving care for long-term stroke and other conditions that particularly affect older people.
The Learning Healthcare System (LHS) is a model of care that integrates data from a variety of sources, develops and utilises decision support tools to manage these data with the aim of personalising and improving the quality of care during clinical consultations. The study aims to design and implement a new LHS for stroke patients, and evaluate whether this system achieves clinical and cost effective improvements in patient care, patient experience and stroke outcomes. The aim of this study is to use methods to engage different stakeholders to jointly input into the development, evaluation and evaluation of the LHS to enable personalised care and improve long term management and outcomes for stroke survivors with multiple long term conditions.
