    Patients admitted to the hospital with cardiovascular diseases (CVD) are susceptible to deterioration and this cause a significant burden in the healthcare sector. We will study the role of a deterioration prediction tool; Early warning score (EWS) in individuals with cardiac diseases. A systematic review on the prediction accuracy of early warning scores in different disease subgroups and different hospital settings and the effectiveness in patients’ clinical outcomes was conducted; which shows a gap in showing the evidence of EWS’s role in patients with cardiac diseases. This has led to the need for studying current early warning score in use – National Early warning score (NEWS2)- in the cardiac patients in different settings. The study will be mixed methods of a quantitative and qualitative study. The quantitative part will examine the predictive accuracy of NEWS2 in St Bart’s hospital of cardiac patients, the accuracy of predicting in a non-cardiac group of patients in St Barts, and the prediction accuracy of NEWS2 in UCLH in cardiac patients where the setting differs. The qualitative part will include a survey of the healthcare staff in use of NEWS2 on the implementation of the tool and its role in the escalation of the care process.
