  • Research summary

Does interventional self-management education based on principles of personal data monitoring, short-term energy restriction and re-balance improve glucose tolerance and health literacy measures amongst newly diagnosed Type 2 Diabetics?
Currently self-management education is recommended for Type 2 Diabetics but is not widely implemented. Existing services focus on recommended population-based guidance and promote the idea that T2 diabetes is a lifelong and progressively worsening condition.
In recent times improved understanding of metabolism and its dysregulation has revealed the potential reversibility of diabetic states for many patients. In addition there is a growing awareness of the need to promote preventive self-management in the context of the growing burden of diabetes and cardiovascular disease on the health service.
The proposed study intends to bring together diabetes self-management education aligned with current standards, personal data tracking of the components of the energy balance equation (weight, activity, diet) and glycaemic control (capillary blood glucose), online support/resources and a personalised short-duration energy restriction and transition diet to demonstrate the impact of behaviour on metabolism.
Based on physiologic principles supported by clinical evidence and built on established theories of behaviour and behaviour change the proposed course meets the criteria for effective lifestyle intervention strategies. 10-12 newly diagnosed Type 2 diabetics (less than 1 year) will be recruited from an existing Diabetes education course and asked to volunteer to attend 6 weekly education sessions of 2-hours each. The course includes aspects of diabetes management and self-monitoring throughout but the 5 week period will be made up of three phases. The first two weeks generate baseline data prior to diet, the second 2 weeks represent 10 days of energy restriction (25-40% of estimated total requirement) and 4 of transition (to 100% of estimated total requirement), and the final week generates post-diet data. Outcomes will be measured in terms of pre and post area under oral glucose tolerance curve and Health Education Impact Questionnaire.
