    Type 2 diabetes is a long-term non-communicable disease of huge societal and economic concern, and it is known that healthy lifestyle choices help in the management of the condition. Research shows that consistent, careful self-management is required for patients to achieve the best outcomes, yet there is currently no unified approach for patients.

    The Diabetes and WELLbeing (‘DWELL’) project involves partners from the UK, Belgium, France and the Netherlands (although this application relates to just the two UK sites involved) and will provide a sustainable co-production programme to enable patients to access tailored support, with site staff, healthcare professionals and patients working together to find personalised, effective ways to manage the condition and improve wellbeing.European partners are seeking ethical approval from their respective RECs.

    DWELL will co-develop a 12-week support programme that incorporates four core areas (education, wellbeing, nutrition and physical activity) and a selection of optional activities to support patients with type 2 diabetes to learn to self-manage their condition. A copy of the outline of the 12-week programme is included with this application (Table 1).

    The overall framework for delivery of the patient support programme will be motivational interviewing (MI), a style of person-centred counselling developed to facilitate change in health-related behaviours. Motivational interviewing is a novel and innovative approach to self-management of type 2 diabetes and will add value in ensuring a tailored and personalised approach for individual participants. Alongside the patient support programme, a training programme for staff and will be devised to ensure effective and consistent facilitation.
    The DWELL project will be evaluated in terms of outcomes for participants, the process, and a cost benefit analysis. This application relates to the evaluation of the programme in two UK sites (Blackthorn Trust (BT) and Medway Community Healthcare (MCH)) in terms of the patient outcomes.
