Research summary
The 'Developing a patient reported measure of care coordination' project is being conducted by the Nuffield Trust, Picker Institute Europe, The King’s Fund, the International Foundation for Integrated Care, and National Voices. The project is funded by the Aetna Foundation.
The aim of this work is to produce a tool that can capture the experience of older people with chronic conditions who receive care across providers. The primary intended purpose of this tool is quality improvement, through enabling a range of care providers to assess where care is fragmented, and to assist with evaluating the success of interventions to improve coordination.
The tool will be developed and tested in England among a sample of service users who are aged 65 or over and who have at least one chronic condition. To test the tool we will recruit up to 6 pilot sites and carry out a postal survey with a sample of 600 patients and service users from each site. Picker Institute Europe are responsible for testing the tool with pilot sites.
The NHS is moving slowly to develop mechanisms to measure integrated care, including patient/user perceptions. It is intended that the tool will provide a consistent and robust measure for providers to use to evaluate care coordination.
