Research summary
This research will identify practices to enable renal professionals to support patients’ decisions between conservative management and dialysis when transitioning from chronic to established kidney disease. Building on our previous highly successful work in this field, we will develop the Yorkshire Dialysis vs Conservative-management Decision Aid (YoDCA) to promote informed decision making between conservative management (CM) and dialysis for patients with ESKD during consultations with health care professionals.
The decision aid will:
• provide health professionals with a resource and training package to facilitate the discussion about the CM/dialysis decision during the consultation;
• improve patients’ experience and participation in their care by providing a tailored approach through the development a theory driven, evidence-based resource;
• increase patient uptake of the most appropriate ESKD treatment option allowing efficient resource allocation;
• lead to patients receiving appropriate management for their ESKD by enabling patient preferences be integrated into care planning.
The work undertaken to produce the decision aid (DA) will lead to at least two peer-reviewed scientific papers on the development of an evidence-based DA for use in practice. The Decision Aid will be informed by guidelines on the development of complex interventions1,2,3. The investigation has 3 parts:
1. A needs assessment3 including: an audit of current patient information and quality assessment of decision aids detailing the CM/dialysis choice, a review of current clinical guidelines and interviews with patients and staff to explore how the CM decision is discussed and staff training needs.
2. Development of the Yorkshire Dialysis vs Conservative-management Decision Aid (YoDCA) for choosing between CM and dialysis treatment for ESKD by synthesizing clinical guidelines and evidence of how to structure information to encourage informed decision making.
3. Prototype testing of a training package for Health Professionals to use the YoDCA in clinical practice by synthesizing guidelines on training health professionals and evaluating the decision aid via focus groups with staff and patients and observing/tape recording the use of decision aids in consultations to provide feedback on resource development.
Wider dissemination and evaluation of the DA will form the basis of future applications.REC name
