    Prisoners have high levels of morbidity. Over 30% of all deaths in custody are due to cardiovascular disease (CVD) and yet preventative approaches to reduce avoidable mortality in these settings are poorly researched. Although community models of CVD risk assessment (Healthchecks) are commissioned in prisons, their implementation and subsequent levels of effective intervention to reduce risk are very patchy. Published data on the systematic pattern of risk factors and uptake is not available to help improve provision of interventions to reduce risk in prison and following discharge. It is not known how the high levels of anxiety and depression in prisoners may further challenge uptake of effective health interventions This study will collect Healthcheck data on 2500 male prisoners, including assessments of anxiety and depression. Contemporaneous existing information will be utilised to minimise unnecessary additional measurement. Analysis will identify patterns of high risk in individuals and groups based on age and ethnicity, including any groupings of physical CVD risk factors, and with anxiety or depression, to help inform multiple risk factor interventions. Focus groups will be used to collect qualitative information on individual and organisational barriers to uptake and supporting better care following release for identified high risk groups
