• Data collection arrangements

  Samples will be stored at King's College London, Faculty of Dental, Oral and Craniofacial Sciences, Guy's Hospital, 17th Floor Tower Wing, Great Maze Pond, London, SE1 9RT. Data will be stored at Guy's Hospital and King's College London on servers. Paper questionnaires and consent forms will be stored at King's College London, Oral Clinical Research Unit, Guy's Hospital, 26th Floor Tower Wing, Great Maze Pond, London, SE1 9RT.

  Potentially suitable new patients will be screened during their hospital visit by their treating clinician (trained member of the biobank personnel team). If the patient is suitable and decides to donate to the biobank, they will be asked to sign the informed consent form. Biobank personnel (i.e. those delegated to take consent, take and process samples) will complete the electronic database with the data collected, will give patients a copy of the questionnaires to complete and will take the samples as appropriate.

  The Dental, Oral and Craniofacial Biobank supports patients attending King's Health Partner hospitals (Guy's, St-Thomas, King's College, Royal Brompton and Harefield hospitals) and Bart's Health Trust in London, UK.
