Data collection arrangements
The type of data to be stored is all routinely collected electronic patient data of adults who have a healthcare contact with Portsmouth Hospitals University NHS Trust PHU either as an outpatient, emergency department attender, day case or inpatient, or who have remote health monitoring, diagnostic or investigative services carried out directly or indirectly by PHU. The data may have been electronically recorded for direct clinical care or may have been recorded as part of a routine audit.There are plans to include data for these patients from other healthcare providers such as that originally collected by GP practices and the South Central Ambulance Service. The objective is to include all electronic records relevant to evaluate outcomes of adult patients (over 16 years old) treated by PHU or at risk of admission.
Care contacts are consecutively sampled without exclusion. Under an existing previous ethics approval (REC ref 21/SC/0080), data is held on admissions from January 2004 onwards.
The research database will be an integrated database of de-identified patient data consisting of demographic and disease attributes, physiological or laboratory measurements, care contacts and interventions along with associated clinical outcomes. A comprehensive list of current data items stored is attached.
Research programme
The research database will contain electronic records relevant to evaluate outcomes of patients treated by Portsmouth Hospitals NHS Trust or at risk of admission with the objective to facilitate the following types of research: - The development and use of algorithms and risk models to identify the deteriorating / sick adult so that interventions and enhanced care can be provided in a timelier manner with consequent resource and outcome improvements. This covers both the deteriorating inpatient at risk of adverse outcomes and people in the local community at risk of hospitalisation or in need of other health services. - The use of case-mix adjusted risk models of adverse outcomes to compare / assess the quality of care given by comparing observed outcomes with those predicted. - The planning of initiatives in healthcare service delivery, including running simulations using the retrospective routine data. - The assessment of the impact of changes in clinical pathways and healthcare delivery to gauge their effectiveness. Other types of research such as those implementing interventions and trials, and those requiring study specific data collection fall outside the generic approval and require separate approvals.
