  • Research summary

    There are approximately 2 million people living with angina in the UK. Angina is chest pain most commonly caused by narrowings in the arteries that supply blood to the heart (coronary arteries). However, nearly half of patients with chest pain have a ‘negative’ invasive angiogram (standard of care diagnostic test), i.e. no obstructions are found in the coronary arteries. In these patients their chest pain may be caused by spasm in the major coronary arteries or problems in the very small arteries which are invisible to the eye. There are tests which can be used to diagnose these problems but these are rarely used in routine clinical practice. Therefore, the diagnosis and management of these patients are uncertain.
    We plan to identify patients with angina and intermediate or no obstructive coronary artery disease seen on a heart CT scan. The patients will be invited to have a standard care coronary angiogram which typically happens for patients with unexplained angina. The research tests involve assessment of coronary function to determine the presence or absence of abnormal function of the coronary arteries to clarify the diagnosis and management of these patients (n=250). These tests are not normally used in routine practice due to lack of clinical evidence. Since the role of coronary function tests to guide management is not well established the study design involves a nested randomised trial with followup. The sample size is 250.
