    Medication is by far the most common form of medical intervention. Medication-related problems are frequently encountered, particularly patients with multiple morbidities requiring multiple medicines and often in the period immediately after discharge from hospital. At that time new and unfamiliar medicines have often been added to their existing medication lists and the patient must assume responsiblity for their care when they return home. The present feasibility project will evaluate patient outcomes (i) after adding a post-discharge telephone follow-up component to the IMM service and (ii) after having a more comprehensive follow-up of patients through a follow-on medicines management programme delivered by the patient’s community pharmacist.
