Research summary
With increasing implementation and uptake of bowel cancer screening, the occurrence very early bowel cancer (T1 CRC), is increasing. Current identification and management of T1 CRCs remains challenging, with existing treatment approaches and risk predictors lacking precision, warranting a comprehensive approach.
T1 CRCs are often not identified accurately prior to endoscopic resection. Furthermore, their optimal management remains uncertain. Current management leads to over-treatment for 80-90% of patients. The benefits of surgery over endoscopic therapy for treatment of T1 CRC are uncertain. The morbidity and mortality of surgery particularly for T1 CRCs in the rectum can be significant with potential for subsequent poor function and quality of life. There is a need for improved identification of T1 CRCs, improved accuracy in identification of those patients that will benefit from surgery and an improved understanding and prediction of outcomes with differing treatment strategies.
We will create a database of clinical factors, endoscopic, pathology and radiology images to improve understanding and accuracy of management of T1 CRCs for patients. We also aim to assess molecular and immunological factors to enhance our understanding of the processes involved to offer potential markers to direct appropriate interventions and identify poor outcomes.
