  • Research summary

This online study is the first of phase one in a doctoral research project. The overall aim of the doctoral research project is to explore the educational needs of the Welsh nursing and midwifery workforce in genomics. Phase one will establish the current context of genomics knowledge and practice within the Welsh nursing and midwifery workforce. This study is one of three which make up phase one. The first objective of this online study is to explore how nurses and midwives who currently care for people impacted by genomic disorders (such as cancer) use their knowledge, skills and expertise in clinical practice. The second objective is to explore what additional educational needs these practitioners have. This will be done by creating a series of clinical exemplars, which are examples of best practice, as well as conducting qualitative thematic analysis. The purpose of the exemplars is to demonstrate the real-word use of genomic knowledge and skills by nurses and midwives in Wales, while the thematic analysis will explore these experiences in more depth as well as identifying any cross-cutting themes.

Method: The research student will perform a series of semi-structured interviews with registered nurses and midwives working in the following specialities in Wales: inherited bleeding disorders, inherited heart conditions, breast/ovarian/bowel cancers, inherited high cholesterol, and midwives screening mothers/newborn infants for inherited conditions. These specialities have been chosen as they are areas of practice in which nurses and midwives use genomics knowledge and skills routinely in the care of their patients. The interviews will be audio recorded. Following the interview, the research student will then transcribe the interview word-for-word to create an interview transcript. The transcript will be returned to the participant to be checked and validated. The transcript will then be used to create a series of clinical exemplars, subject to the participant’s consent at that point. Each exemplar will be a short narrative summary of the interview under the heading of the clinical area. Direct quotations from the interview may be used to illustrate specific points. The purpose of the exemplars is to explore how nurses and midwives currently use their genomic knowledge, skills and experience and to explore what educational needs the workforce has.

Qualitative thematic analysis will be conducted on the interview transcripts to identify any cross-cutting themes to inform the development of educational resources and continuing professional development of the nursing and midwifery workforce in genomics.

The research student will perform a series of semi-structured interviews with registered nurses and midwives working in the following specialities: inherited bleeding disorders, inherited cardiac conditions, breast/ovarian/bowel cancers, inherited high cholesterol, and midwives screening mothers/newborn infants for inherited conditions. These specialities have been chosen as these are areas of practice which use genomics and genetics routinely in the care of their patients. The research student will then transcribe the interview word for word to create an interview transcript, which will be checked and validated by participants. The transcript will be used to create a series of clinical exemplars, which are examples of best practice. The exemplar will be a short narrative summary of the interview. Verbatim quotations may be used to illustrate specific points. The purpose of the exemplars is to explore how nurses and midwives currently use their genomic knowledge, skills and experience, and to also explore what educational needs the workforce has.
