Research summary
Chronic nonbacterial osteomyelitis (CNO), also known as chronic recurrent multifocal osteomyelitis (CRMO), is an autoinflammatory bone disease of unknown cause. Its diagnosis is made by excluding alternative diagnoses. CNO is a heterogeneous disorder with CRMO as its most severe form. The Childhood Arthritis Research and Rheumatologist Alliance (CARRA) CRMO workgroup has developed consensus treatment plans for children with CNO refractory to nonsteroidal anti-inflammatory drugs (NSAIDs) and/or with spinal lesion (Zhao et al. 2017). Future observational studies and clinical trials are needed to identify efficacious treatment for this condition. Inclusion of well-defined patient groups is required for future research studies. Diagnostic criteria for CRMO have been proposed by Jansson et al. (2009) and Roderick et al. (2016). Both criteria were derived from cohorts in a single center, and neither criteria has been validated. Thus, a set of classification criteria that is sensitive and specific to identify candidate patients needs to be developed using a data-driven approach. Furthermore, validation of developed classification criteria and previously published diagnostic criteria is crucial before applying these criteria in research and clinical practice.
