  • Research summary

Determining the appropriate target weight for children receiving dialysis is a significant challenge given the lack of reliable objective assessment techniques for total body fluid status.

We hypothesise that that chest ultrasound examination can be used to assess fluid status in children requiring dialysis, and guide the assessment of their target weight.

The objectives of this single centre study are:
    1. To compare lung ultrasound measurements with currently available measures of patient fluid status including patient weight, blood pressure, clinical assessment, bioimpedance measurements and echocardiogram findings.
    2. To develop a lung ultrasound B-lines score to quantify to quantify fluid overload in children receiving dialysis.
    3. To determine if lung ultrasound findings correlate with symptoms of fluid overload or depletion such as breathlessness, headache and abnormal blood pressure.

    Methods:
Children aged 0 – 18 years receiving haemodialysis or peritoneal dialysis will be eligible for the study.
Brief chest ultrasound examinations will be performed before and after dialysis with a sonosite S-ICU ultrasound 2-5MHz curved probe on the skin. The number of "B lines" in the midaxillary, anterior axillary, midclavicular and parasternal spaces will be measured.
B-line numbers will be compared to clinical parameters of fluid status including patient weight, blood pressure, clinical assessment, bioimpedance measurements and echocardiogram findings. If significant correlation of B-line numbers with clinical parameters is found, a B-line score will be proposed.
