    Sarcoidosis commonly affects the lungs, and in some patients it is persistent or progressive. Two key unmet needs are identification of the foreign proteins (antigens) that trigger sarcoidosis, and understanding the florid immune response to identify better, more specific treatments. Our proposed research is based on observations from our laboratory that abnormalities in the monocyte (a blood cell involved in inflammation) are key in sarcoidosis. We have found that abnormal CD200R (a protein on the surface of the cell) could explain the inflammation in sarcoidosis because CD200R normally acts to reduce the immune response. We suspect that measuring CD200R could be used to identify early on whether sarcoidosis is likely to worsen or improve. We will investigate this in a cohort of sarcoidosis patients by undertaking regular immunological assays including monocyte CD 200R expression and correlate these with clinical markers of disease progression over 1 years follow-up. In addition we will undertake a series of laboratory based basic science studies examining various aspects of monocyte function and immune function.
