Research summary
The rise of long-lasting illnesses among older adults is becoming a big worry because it affects their personal well-being and puts a strain on society. More and more older people are dealing with two or more of these long-term health issues. In fact, three out of four seniors in developed countries are facing this.
To help these older adults effectively, we need to treat them individually because each person's health needs are different. That's why the CAREPATH project was funded by the European Commission. This project aims to create a digital health system that can manage multiple long-term illnesses in people with mild cognitive impairment or mild dementia.
The CAREPATH system has three main parts that work together:
i. Home/Health Monitoring Platform (H/HMP): This part uses special devices like sensors and smart gadgets to collect health information from people. It looks at things like how movement in and around the home, how patients sleep, and even some health information like blood pressure and heart rate. All this data gets sent to the system for checking where it can be viewed by a patients healthcare team. All the devices used by the CAREPATH system are ones you can currently buy and use, and they're all checked to be safe (CE marked). They're connected using a special device made by one of the project's technical friends (Eclexys).
ii. Patient Empowerment Platform (PEP): This is an app on a tablet which we will provide patients with. The apps features will help patients record and view information important for their health. It will also let patients talk to their doctors and nurses, and they can even play memory games and do certain exercises.
iii. Adaptive Integrated Care Platform (AICP): This is the part of the system used by the healthcare professionals involved in enrolled patients care. It lets the healthcare team make special plans for each person. It uses info from the H/HMP, PEP, and even electronic health records to make these plans. That way, everyone gets the right care for them.
To make sure this CAREPATH system works well for its main users - people with mild cognitive impairment or mild dementia, their family members who take care of them, and doctors – we will perform a technical validation and usability study. This study will happen near the end of CAREPATHs development. It will focus on finding any problems in using the system in the real world, with the people who are supposed to use it. The things we learn will help us make the system better before we do a larger follow-up clinical trial.
To perform this technical validation and usability study we will need 4 patients and the informal caregivers, and 4 healthcare professionals who will use the system for 3-months and provide regular feedback on their experiences. Feedback will be provided in different ways including telephone and face to face interviews, questionnaires, and remote monitoring of the systems use. The CAREPATH system will supplement, and does not replace, standard patient care.
