    There is a growing number of people across Europe living with heart failure (HF), yet limited healthcare resources are available to meet all their needs. SanaCoach Heart Failure (SanaCoach-HF) is an online application that will provide information to patients on their heart failure condition, ongoing treatment and required lifestyle modification, enabling them to be self-caring. Patients who are treated by the heart failure community service teams and CHESS (i.e., Care Home Engagement and Support Services) will be screened for eligibility using available clinical data. Potentially eligible patients will be identified by the clinical team who will pass their contact details to the research nurse, who has an honorary contract with the hospital. The research nurse will invite them to the study.
    Consented participants will use the SanaCoach-HF application, in addition to standard care for the duration of at least 6 months. The main aim of this study is to establish whether patient-centred digital decision systems like SanaCoach-HF may be administered to patients as a Web app to improve patient self-management and lifestyle among patients with chronic heart failure. Data will be collected on patients' use of the device, their satisfaction as well as quality of life, self-care and adherence over the 6 month period.
