    Central airways obstruction (CAO) affects up to 30% of lung cancer patients. There is currently a wide variation in management of this problem with little evidence to guide patient selection for intervention. The benefit of intervening in proximal CAO that potentially causes asphyxiation is difficult to dispute but the value of intervening in more distal, lobar obstruction is less clear. We aim to prospectively study lung cancer patients and compare symptoms and quality of life in those with no CAO versus proximal CAO versus distal airways obstruction. Quantifying symptom burden in each of these situations can further our understanding of when intervention is appropriate. If we establish comparable symptom burdens then this would justify further research looking at the benefit of intervening in distal CAO.
