    Life expectancy of people with chronic diseases has increased significantly in many countries due to advances in medical science, among other things. Health services face new challenges such as ensuring that the patient lives as independently as possible, providing support to caregivers, who bear an increasing workload, and creating sustainable health and social systems. The ADLIFE project aims to overcome these challenges by providing integrated, coordinated, anticipated and personalised care to people with advanced and chronic diseases who face a potential reduction in life expectancy due to decreases in functionality and capacity. During the four-year ADLIFE project, a digital toolbox will be developed to improve the quality of life of patients with COPD and / or heart failure as the main diagnosis. The toolbox will promote the active participation of patients in their own care plan, e.g by planning their care jointly with healthcare professionals. The ADLIFE system will be deployed through pilot projects in seven countries and health services that include NHS Lanarkshire.
    As a preliminary stage of the ADLIFE research project, each of the seven regions where the new system will be piloted, will carry out a survey by means of qualitative methodology (interviews and focus groups) among key users to analyse the particular care model and the level of integration in terms of care for patients with advanced chronic diseases (COPD and / or heart failure). The objective of this study is to picture the current care model in each of these seven regions and what are the possible areas for improvement in each of them. The information obtained from this proposed study will be the starting point on which each region will later begin to prepare for the implementation of the new digital tools in their local systems, within the framework of the ADLIFE project.
