    The focus of this project is to map the patient journey from referral to discharge and integration back into the community following a spell of bed based rehabilitation and what impact it might have had on them either functionally and/or their activity of daily lives . The service user and/or their carers should be the main beneficiary of the service. However, the perception among our Therapy staff in particular, is that Intermediate Care services such as us are not service-user driven. It is to this extent that this project would explore a personal outcomes approach to the patient journey. This would assist organisational service development, as well as highlighting recent government policies such as personalisation and enablement agendas, among others.
