Research summary
Musculoskeletal physiotherapy (muscular, bone and joint problems)is commonly experienced by many patients in the NHS and private sector. As part of MSK physiotherapy consultations, patients are usually asked to complete a physiotherapy programe which requires them to carry out exercises or make changes to their usual behaviours. A recent literature review shows that approximately only half of patients adhere to their physiotherapy programme and that those who do adhere have better outcomes. Little is known about the amount of information given in physiotherapy self-management programmes(SMPP)and if this affects if people adhere. The COM-B (Capability, Opportunity and Motivation-behaviour)behaviour change model is based on the theory that for people to perform certain behaviours they must have the capability, opportunity and motivation to do this behaviour. This model has not been evaluated in physiotherapy. The COM-B model will provide a framework for this research which will used mixed methods to explore the content of SMPP and patient's experiences,behaviours and intentions to adhere to their programme.
It will recruit patients attending two North West NHS physiotherapy services. Video recording of usual care consultations will be made of the first and follow up physiotherapy consultations. Patients will be interviewed after their appointments regarding their physiotherapy programmes and their intentions and behaviours to adhere and experiences of carrying out their self-management physiotherapy programmes. Patient participants will also be asked to complete a short survey regarding their opinions, behaviours and intentions towards their SMPP at 3 time points. This research aims to explore patients' behaviours and intentions using an established psychological model that will lead to better understanding of individual's behaviours and barriers and motivation towards completion of physiotherapy programmes. This will inform the development of this model and the design of interventions to increase adherence and facilitate behaviour change, thus improving effectiveness and efficiency of physiotherapy interventions.REC name
