    There is currently a focus towards the healthcare needs of the oldest old at the end of life, with the support/care received towards the end of their lives being recognised as an important component of high quality health and social care. For the purpose of this study the oldest old is defined as people over 85years.

    The oldest old are the fastest growing sector of our population. They are more likely to have complex health needs, multiple illnesss constitute the majority of care home residents and are more likely to spend their final year in poor health than people who die at younger ages. Overall, the oldest old receive poorer-quality care towards the end of life than younger people. They are, less likely to be involved in discussions about their options, less likely to die where they choose, less likely to receive specialist care.

    Nurses can use a special check list to help them estimate if a patient is likely to die over the next year. This is called the gold standard framework prognostic indicator and is important as it means that nurses can make sure the patient can get the care they need in the place they need it.

    People over 85 years identified to be in the last year of life will take part in in three interviews at six week intervals in order to explore their care needs and preferences and identify if these change over time. I will also review case notes,to obtain an enhanced picture of care provided from a healthcare perspective. I will provide patients and relatives with a disposible camera to determine what home means to them. Following the interviews I will have two focus groups with relatives and healthcare professionals to provide further information to support the exploration of needs and preferences.
