Research summary
Aortic stenosis is a narrowing of the main valve controlling blood flow out of the heart and is the commonest heart valve disease. When it becomes severe and causes symptoms such as breathlessness, it needs to be treated by replacing the valve, otherwise if left, this can lead to heart failure or death. Optimising the timing of valve replacement surgery is important for improving patient outcomes, and this relies on accurate assessment of the severity of the valve narrowing and whether the breathlessness is due to the valve, or lung disease. In many patients, uncertainty exists in these evaluations, particularly concerning the cause of the breathlessness. Our study will adopt detailed imaging approaches that may help resolve some of these uncertainties to optimise clinical management. Cardiac magnetic resonance imaging, computed tomography and Xenon lung imaging will be used to help define the severity of the aortic and lung disease more accurately and differentiate the symptoms to allow optimal management. Magnetic resonance imaging also has unparalleled ability to evaluate the response of the heart muscle and could allow early identification of those patients starting to decompensate. Lastly, complex flow patterns around the heart, which can now be assessed with magnetic resonance, may aid the planning of surgery in those patients needing valve replacement.
