Research summary
This four-phase study seeks to provide a deep understanding of how care planning works in primary care, by asking the questions: what works, for whom and in what circumstances? Phases 1 and 2 have been completed, and phase 3 will commence in April 2017.
Phase one was a review of the literature in care planning. From the literature, seven theories have been formulated, that make sense of the processes underlying care planning.
Phase two incorporated a focus group with 'experts' in care planning. During the focus group, the participants were presented with the seven theories (in the shape of if... then... statements) that were formulated in phase one. During a discussion, they were given the chance to change or add to the theories.
Phase 3 involves testing the refined theories in focus groups by identifying whether, how, for whom and in what circumstances care planning works in practice, from the perspective of healthcare professionals.
Phase 4 will involve testing the refined theories in GP practices across the North East of England, from the perspective of patients. Semi-structured interviews will be used to allow for participant views to be explored. Approximately 10 patients with one or more long term condition(s) will be invited to participate in the study.
