We would be grateful if you would take a few minutes to provide feedback on your experiences of being a Member/Officer of a REC/CAG, based in England or a Devolved Administration. We are particularly interested in any suggestions you may have, on how we can improve your experience of being a member of one of our committees, or to the researchers applying for approval. 

Completed forms will be shared with the appropriate HRA or Devolved Administration staff according to the feedback you have given and will include your name and contact details.  The staff member will liaise directly with you to discuss your feedback and will share this response with HRA QA.

Feedback will be used both individually, to inform local situations and specific areas of business, and in a collated form to assess trends.  A summary of feedback received is included in the Bi-Annual User Satisfaction Report (available on the HRA website).
